Health Care Directives In-Person

A health care directive is a written document that informs others of your wishes about your health care. This program will equip you with the knowledge and resources you need to create your directive—and help ensure that decisions are based on your values and preferences, even if you are unable to make them yourself. This program is presented in partnership with Trellis, the Twin Cities Metro agency on aging.

Trellis provides services, connections and innovations to help people thrive as they age. Through partnerships with community organizations and healthcare organizations, they work to bridge social care and medical care to produce more positive outcomes for individuals and communities. They are committed to reducing disparities in access and ensuring equity and inclusivity.

Anoka County Library strives to ensure that all visitors are able to access library resources, services, and programs. Assistive technology and program accommodations are available at many library locations as outlined here. Please call the library branch hosting the program at least two weeks prior to an event if there are additional accommodations we can provide for you to fully participate.
